       –    Injured Worker
       –    Employer Location
       –    Physician
  •   Obtains Job Description
  •   Sets Strategy with Adjuster and Employer

Our nurse will assess:
      –   Mechanism of Injury
      –   Severity of injury
      –   Ability to return to work
      –   Goal oriented treatment plan to return to work
      –   Impact of pre-existing/non-work related injuries or conditions on return to work
      –   Inconsistencies that may affect return to work

  •         Return to Work as quickly as possible
      –        Minimize or eliminate lost time
      –        Assist you with OSHA issues
  •         Establishment of goal oriented treatment
  •         Identification of issues affecting return to work
  •         Coordination of information to all parties
  •         Coordinate referrals to appropriate specialties and sub specialties
